Welcome to the Clark School Significant Accomplishments Submission Form!
By using this form, you can send us your most recent accomplishment(s) at any time throughout the year for inclusion in our semi-annual online publication, Significant Accomplishments.

You are asked to describe your accomplishment using the fields below and then to submit a brief write-up. Filling in the fields will help us sort and prep your entry and can help you make sure your write-up includes all of the necessary information. If you have any questions about how to use this form, please send an email.

Please note that not all entries are guaranteed to make it into Significant Accomplishments due to space considerations. Patents, published papers and grants under $1 million typically are NOT included. We reserve the right to edit all entries. We may contact you if we have questions about your entry.

We are currently accepting entries for the Fall 2009 semester. Entries for this edition must be submitted by November 30. The Fall 2009 edition of Significant Accomplishments will then be sent out in January to Clark School alumni and friends.
