Changing Major

Internal Major Change (online form)
This form is for a student changing their major within the A. James Clark School of Engineering only. If you plan to change out of the Engineering School, contact the corresponding department.

There will be a delay on the processing of Change of Major request. There is no need to resubmit requests, they will be process in the order they came in.

LEP-Appl-ENGR (online form)
If you are a student in another college who wishes to apply to the           A. James Clark School of Engineering. Student needs to complete the gateway courses and complete the Limited Enrolment Program (LEP) Application at the above link and submit the application electronically.

Exceptions

Petition for an Exception to Policy (pdf)
The Clark School of Engineering takes request for exceptions extremely seriously. We will consider exceptions to academic policy only in rare and extraordinary circumstances. Be sure to attach a typed statement and include supporting documentation on department letterhead or e-mail. Print, sign and date the front of the exception form and return to 1131S  GLMH.

Appeal of Petition for an Exception to Policy (pdf)
If your petition was denied, you may appeal the decision.  For strongest consideration, all documentation must accompany the appeal form at the time of submission. Return applear form an additional information to 1131T Student Affairs/Associate Dean's Office.

Graduation

Degree Navigator
Students with 90 or more credits will be held responsible for reviewing their Degree Navigator (DN) unofficial online audit with their Departmental Advisor.

Embassy Letter Request (pdf)
Upon successful completion of all remaining courses and meeting all University requirements, a student can submit a request for verification of anticipated graduation for an embassy to allow parent(s), guardian(s), or other family members to attend the Engineering Commencement ceremony. Return form to 1131S  GLMH.

Anticipated Letter of Graduation for the FE/Fundamentals of Engr. Exam (online form)
It is the student's responsibility to submit an FE application, to request an official transcript from the Registrar's office and have it sent to the state board office. Provide a valid social security number to the state board office. This request needs to be submitted no later than 5 business days prior to the FE application deadline. This form is for Undergraduate Students Only. If you are a Graduate Student, please contact your department. Please do not submit this form until January for the April FE exam date or June for the October FE exam Date.

Graduation Letter Request to Employer (online form) THIS REQUEST IS FOR EMPLOYERS ONLY.
The Graduation Letter Request can only be processed upon successful completion of all remaining courses and meeting all University requirements. This form is for Undergraduate Students Only. If you are a Graduate Student, please contact your department.

Graduation Worksheet (pdf)
The Graduation Worksheet is to be completed the semester that you actually graduate. LATE Graduation Worksheets need to be submitted with a Petition for an Exception to Policy form found http://www.eng.umd.edu/advising/advising_forms.html.

Make sure to Apply for Graduation, complete the Senior Survey and return all above forms to 1131S GLMH.

Request to Walk in Engr. Commencement Ceremony (online form) Last Day to submit request is December 11, 2009 by 4:30pm.
Request to Walk (online submission). Students who will complete final coursework towards their degree program in the summer or winter term, but wish to participate in the Fall or Spring Commencement ceremony should complete this form. Participating in the graduation ceremony does not guarantee graduation from the university. Please do not submit this request until November 20, 2009.

Request to take Graduate Courses

Combined Bachelor's/Master's Degrees Form (pdf)
With the approval of the Department and completion of program requirements, an undergraduate degree-seeking student can request to join the combined bachelor's/master's (BS/MS) program. In the BS/MS program, graduate level courses initially taken for undergraduate credit may also be applied towards the graduate credit requirements for a master's degree program at the University of Maryland. Return form to 1131S GLMH.

Graduate Credit Permission Form (pdf)
An undergraduate degree-seeking student with approval can apply to take a course for graduate credit only.

Steps to follow when completing form:

  1. Top part to be completed by student
  2. Department Certification- to be signed by student's major Department advising dept-contacts
  3. College Certification- to be approved by the School of Engineering (1131S GLMH)
  4. Graduate School Review- to the Graduate School for approval (2133 Lee Bldg.)

Permission to Take a Graduate Course for Undergrad. Credit (pdf). Complete this form if you wish to take a graduate level course and have it count for undergraduate credit.

Steps to follow when completing form:

  1. Top part to be completed by student
  2. Departmental Academic Advisor should sign to give permission and denote what the course will count for in the degree
  3. Professor of Grad-level course should sign to give permission
  4. To 1131S GLMH Undergraduate Advising & Academic Support
